CareMedica participates with most insurance carriers. We do, however, require your insurance information prior to your appointment to verify eligibility. If you have questions regarding your insurance coverage prior to your visit, please call your insurance company and our billing office (203-672-2800 x8). We are unable to waive co-payments due to contractual terms with our carriers. Thank you for your understanding.
Please be prepared to show your insurance card at every visit as well as a form of ID. The latter is to protect your financial information.
We will see you as a self pay patient. Please speak with our billing staff prior to scheduling your visit.
Co-payments and balances
Co-payments are the patient’s responsibility and are to be paid at the time of service. Any patient balances are expected to be paid when invoice is received or within 30 days. If you need to set up payment arrangements, please contact the number above for our billing office. Payments may be made by cash, check, VISA, MasterCard, AMEX, or Discover.

If your visit requires laboratory tests, x-rays, special x-ray studies and other services done outside of our office, a referral may be required by our physicians. CareMedica will provide these specialists and facilities with your billing information. Our staff will contact your carrier when it is required. Patients are ultimately responsible for knowledge specific to their individual plans so we ask that you take an active part in knowing your plan requirements.
